Eligibility Requirements:
To enter the Mother’s Day raffle, you must be a mother or are preparing for parenthood to meet criteria.
Must be available for an in-person appointment at Move Meant Physical Therapy, INC in Fullerton, CA
If chosen, you must fill out onboarding documents (i.e. health history forms et al) prior to appointment.
